大多数人都是这样磕磕碰碰学习过来的。

medical necessary 肯定会 （eventually） approved的，除了少数exception. 不少的时候 pre-auth需要back and forth multiple times，才能搞定. been there done that.

这个就是保险这个制度的弊病之一。如果是想中国一样，基本看钱，就少了一大部分扯皮的事。
当然，我宁愿 deal with 扯皮这些 hassle，也不愿啥都是简单付钱搞定，但这个就是 trade-off.

关键的是你没办法等。不知道是啥原因，如果是medically necessary now, 找医生出证明 and then appeal. 但如果是医生认为 necessarily but not needing to rush, 只是你不愿意等，那这个就是“提前”（没走完流程）的代价。

yeah, we all need to learn how to work the system, though not always in the way that i like. but that is life. 以前家里有人需要重要的药，我 pre-auth back and forth 差不多一个月才搞定。病情明显给影响了，没办法。没钱只能这样。

manage your expectation. low expectation = low disappointment.

still, u should appeal to the right places (google to find where to appeal correctly, and there is a correct process for that you need to follow), and do not expect a lot. maybe u will be surprised?

是的，必须自己 take care. can’t just trust them, b/c they do not have your best interest in mind. if you do not take care of yourself and your family, no one else would.

kaiser: i think kaiser is very good for general care, 就是普通病。但是专科都是比较差的，听说。不知道是不是kaiser的薪水比较低，或者不愿意花钱请多些专科，so that specialists are either lower quality or just understaffed. 不过只是道听途说。

我现在正打算给我的急诊账单appeal 但是急诊医生office说他们不出这种证明怎么破？我就只能自己干写？

你如果知道医生名字，可以看如果在ER之外找到他？不太清楚，反正appeal都是你找东西说，怎么有理有据怎么写呗。看作文水平和google水平了。

多年以前，为了家人，曾经堵医生，烦到他答应。不过那不是为了钱，而是为了病情不等于而为之，也不堪回忆。一般不建议。
