medical necessary 肯定会 (eventually) approved的,除了少数exception. 不少的时候 pre-auth需要back and forth multiple times,才能搞定. been there done that.
这个就是保险这个制度的弊病之一。如果是想中国一样,基本看钱,就少了一大部分扯皮的事。
当然,我宁愿 deal with 扯皮这些 hassle,也不愿啥都是简单付钱搞定,但这个就是 trade-off.
关键的是你没办法等。不知道是啥原因,如果是medically necessary now, 找医生出证明 and then appeal. 但如果是医生认为 necessarily but not needing to rush, 只是你不愿意等,那这个就是“提前”(没走完流程)的代价。

manage your expectation. low expectation = low disappointment.
still, u should appeal to the right places (google to find where to appeal correctly, and there is a correct process for that you need to follow), and do not expect a lot. maybe u will be surprised?
