因为属于比较少有的remote员工,所以公司大概懒得搞好的plan,现在提供的plan非常奇怪…简直不知道哪个叫High deductible的程度,但是因为选PPO的话同时会有$1500的employer contributed health reimbursement,好像刚好卡在我medical expense上,所以我就纠结上了![]()
PPO:
Premium差不多一年$2000,然后plan details:
- Deductible: $3,500 individual / $7,000 family
- Out-of-Pocket Max: $6,350 individual / $12,700 family
- Primary Care Visit: $30 copay
- Specialist Visit: $50 copay
- Urgent Care: $30 copay
- Inpatient Care: 30% after deductible
- Outpatient Surgery: 30% after deductible
- Emergency Room: $150 copay then 30% after deductible
- Prescription Drugs: Varies ($5/$20 copay for generic, $40 for brand, $60 for non-formulary)
- HRA Benefits: Employer contributes $1,500 for individual
- FSA
HDHP:

- Deductible: $1,600 individual / $3,200 individual in family / $4,000 family
- Out-of-Pocket Max: $4,000 individual / $8,000 family
- Primary Care Visit: 20% after deductible
- Specialist Visit: 20% after deductible
- Urgent Care: 20% after deductible
- Inpatient Care: 20% after deductible
- Outpatient Surgery: 20% after deductible
- Emergency Room: 20% after deductible
- Prescription Drugs: After deductible, $5/$15 copay for generic, $40 for brand, $60 for non-formulary
- HSA
两个plan的network都一样。总之就是两个都很不咋样但是还是需要选一个相对好点的
我自己除了每年体检以外,明年还预计要看一下皮肤过敏的问题,估计要去几次诊所。今年选的是PPO + FSA,看了下大概medical expense是$1200,大头其实是self-pay的eye exam和两次根管(垃圾Guardian不给钱,眼科牙科医生全都不收)花了$750。预计明年可能也是这个$1000 - $2000的区间?
现在看HDHP的premium便宜600块,但是选PPO的话公司先给1500;不过如果expense上去了,PPO的out of pocket max就比较多辽…有点算晕了orz目前倾向于还是PPO因为感觉medical expense超过600还是很有可能的,但是不知道有没有漏掉啥没考虑到的情况
